State Board Information

State Board Information pertains to current undergraduate students who are in their last semester and will graduate May 2008. If you are uncertain as to whether or not you will be a May graduate, please contact your advisor at (608) 263-5202 to check on your student status. The degree date for May graduates will be May 18, 2008. This is the date that will be printed on your official transcript.

Degree Certification:

Most board of nursing offices require you to document your nursing education. Documentation varies by state. The most common methods used for documentation are an official transcript, a degree verification form or by both methods combined.

Applicants applying to:

  1. Wisconsin Board of Nursing (http://drl.wi.gov/prof/rn/cred.htm ):

    A degree certification letter for all UW-Madison undergraduate nursing students is automatically delivered to the Wisconsin Board of Regulations & Licensure as soon as the nursing degree has been confirmed. Therefore, students do not need to contact the UW-Madison School of Nursing to inform the school that they are applying to the State of Wisconsin.

    If students wish to check on the status of their WI application, go to the Department of Regulation and Licensing Online Application Query web site. The URL is: http://webdrl.state.wi.us/replmain/web_ats_query.

 

  1. State Board of Nursing (other than Wisconsin)

Students are responsible for providing Degree Verification forms to the School of Nursing Academic Programs Office, room K6/146. Once the forms are completed, they will be returned to the the appropriate address.

    A booklet that lists all of the Board of Nursing offices within the United States and their addresses is available in K6/146.


The state of California requires that all applicants for licensure have taken bacteriology lecture and lab to be eligible for licensure in that state. This requirement holds whether an applicant is going to take State boards in California or applies for licensure by endorsement.

In the state of New York, graduates of nursing schools outside of that state have a couple of extra hoops to go through when compared with graduates of their instate schools. Applicants for licensure are required to present evidence of completion of a two hour course in the identification and reporting of child abuse. This course work must be obtained from a provider specifically approved for that purpose by the New York State Education Department. Once a job has been located, the employer must fill out a section of the applications for a work permit; on that application it states that it will take 30 days to process the work permit and that a graduate nurse cannot begin work until that permit is received.

Temporary Work Permits-Wisconsin:

A permit is granted for a three month period and is issued upon the following:

    1. Completed state board application accompanied by the $57.00 fee

    2. Permit fee of $10.00- (therefore a total of $67.00 is required)

    3. Official certification of graduation from a state-approved school of nursing. The certification is not acceptable until after the program has been completed.

NCLEX review course information is listed below. Additional materials will be placed in the Student Communications Room, K6/162 as they are received.

A demonstration of the computer adaptive testing format (CAT licensure exam) is available in the Simonds Center. Choose the menu selection CAT NLN DEMO to try it out. We encourage all students to do this.

An NCLEX-RN Examination Review Course is now offered via the World Wide Web (http://www.nclex.com). This web based review course provides nursing students with the opportunity to use it 24 hours a day seven days a week with the most up-to-date course work available.

The cost to connect to this comprehensive course is $249.00 for 15 weeks of unlimited personal access. Along with the National Council's Review Course, is a direct link to Taber's online encyclopedia of over 55,000 nursing/health care terms.

NLN DIAGNOSTIC READINESS TEST FOR RN LICENSURE

The Diagnostic Readiness Test for RN Licensure is offered by NLN to help graduating seniors and graduates prepare for the National Council Licensure Examination for Registered Nurses. The Diagnostic Test is based on the NCLEX-RN test plan and assesses the knowledge and skills needed for entry-level practice as measured by the licensure examination. This test includes 172 items and requires three hours for administration.

Scores are reported in the form of an individual Performance Profile which is mailed directly to the student within 48 hours after the completed answer sheet is received at NLN. The Profile provides an estimate of the Student's probability of success on NCLEX. In addition, the Performance Profile indicates the student's relative strengths and weaknesses in several categories.

A customized study guide is included to tailor the student's review program. Specific text references are noted for any areas of weakness demonstrated by the student. Students are also given correct answers and rationales for each item.

Official Transcripts:

The UW-Madison Office of the Registrar offers a web-based system to order and pay for official transcripts.  Students can place transcript orders at ordertranscript.wisc.edu, or by access through MyUW (go to the Student Center, and click on “Order Official Transcripts”).  The typical time for processing a complete order request will be 1-3 days. 

The UW-Madison Office of the Registrar also offers in-person ordering (at the computer annex) and pickup at our office, located at 21 N. Park Street, Suite 7223.  Full payment will be required at the time of service.

Payment for transcripts will be accepted online using a credit card (Visa and MC), by mail using a check, or in person with cash or check (payable through the Bursar’s Office).

The State of Wisconsin Department of Regulation and Licensure has a web page where you can go and check on the status of your application once you have applied.
