Student Evaluation of Instruction Information

Evaluation Form | Evaluation Period | Instructions | Special Notes | FAQ

Student evaluation of instruction is an important evaluation tool because faculty often make changes in courses on the basis of student feedback. The statistical data obtained from the completed evaluations are also used as part of the faculty merit process. It is extremely important that all students complete the on-line evaluation for each of the School of Nursing courses in which they are currently enrolled.

Some courses, (e.g., N105, N601) have a discussion, clinical or laboratory component in addition to the lecture. Students will evaluate both components of these courses. (Note: Clinical -N419 students will evaluate only the Lecture, not the individual labs. N219 Students will evaluate only the labs and not the lecture. Students enrolled in the following courses do NOT have an evaluation form to complete: N341, N406, N434, N437, N444, N449, N490.)

The evaluation consists of 23 multiple-choice questions, which are divided into 5 topic areas: 1)Course Administration, 2)Instructor, 3)Grading, Courses Assignments and Reading Material, 4)Outcomes and 5)Overall. There are 3 open-ended questions.

Evaluation Period:

The evaluation period will run from August 5 through August 15 *. Anytime during this period students can go to the SoN Student Evaluation of Instruction form and complete an evaluation for each School of Nursing course in which they are currently enrolled. The evaluation form can be accessed twenty-four hours a day, seven days a week. Internet Explorer is the recommended browser. To log in to the Evaluation form, students will be asked to use their email address (that is the address the school used to email students regarding the on-line evaluation process) and their date of birth in MM/DD/YYYY format.

Special Notes:

If a specific course had a different evaluation period, the information would be listed below.

*Course Eval for N614, N615, sections 60:

The course evaluation form will be available from August 22 through August 29.

*Course Eval for N302, N701, N704 and N870:

The course evaluation form will be available from July 1 through July 15.

*Course Eval for N322 and N605:

The course evaluation form will be available from July 8 through July 22.

Step-by-Step Instructions:

Download a PDF format of the step-by-step instructions. (This link requires Adobe Acrobat to open. You can get Acrobat for free by going to www.adobe.com ) These instructions provide students with information on how to sign on and complete the evaluation form.

Cross Listed Courses:

The following courses, 105 and 746, are cross-listed courses. This means that students other than nursing students are registered for these courses. We are asking all of the students who are currently taking one of these courses to complete this on-line evaluation form, whether or not they are a Nursing student. The responses to the questions will remain completely anonymous. A statistical summary of the results together with a printed summary of student comments will be given to faculty after final grades have been submitted for courses.

Collaborative Nursing Courses:

Faculty throughout the state teach CNP courses, however, we want students to evaluate only the courses that are taught by UW-Madison School of Nursing faculty. For Spring 2008, the NCP courses students need to evaluate are: N306, N317 and N519.

Independent Study:

An on-line student evaluation of instruction form is provided for independent study. For the most part, faculty generally have no more than one student per semester participating in an independent study, therefore, we feel it is important that we inform students who complete the evaluation that their submission may not be anonymous.

Frequently Asked Questions:

Question #1 - I have tried to access the evaluation form, but it won't accept my email address. What do you recommend?

Answer: The problem seems to be the email address. Because of the change over that took place at DoIt this summer with email, it affected some email extention. The old email addresses ended with @students.wisc.edu, the new email extention has left out the word "students". So try your email address with @wisc.edu and see if that logs you in.

Question #2 - There were two evaluations for the same course, one for lecture and one for discussion. The professor was the same for both lecture and discussion. Do you need me to do it for both surveys or is one sufficient?

Answer: Both evaluations need to be completed even if the instructor is the same. You are being asked to evaluate both the lecture and discussion separately.

Question #3 - I am a collaborative nursing student who does not have an email from the school. I never received one. I use my own email. I would still like to fill out the on-line student eval but won't be able to unless you have some suggestions.

Answer: No problem here, just use your current email address and your birthdate, MM/DD/YYYY format.

Question #4 - When do faculty receive the results of the completed evaluations?

Answer: A summary of the completed evaluation is distributed to faculty about a month or so after final grades are posted. Faculty are never given individual evaluation forms, only a a summary report.
